Unshaven Hair Transplant

An unshaven hair transplant is an approach in which the hair is not completely shortened, or only a limited section is shaved. It is especially attractive to women, men with longer hair and patients who want to return to everyday life with a less obvious change in appearance.

How Is an Unshaven Hair Transplant Performed?

The pattern of hair loss, donor density, estimated graft requirement and hair length are assessed first. A shaving plan is then made, grafts are harvested individually with a micromotor technique and, in our practice, they are most often implanted with DHI.

What Does “Unshaven” Actually Mean?

Partial-shave hair transplant

Only a small area of the donor region is shortened and this can often be concealed beneath longer hair.

Recipient area left unshaven

The donor area may be shortened while the existing hair in the recipient area is preserved at its normal length.

Hidden shave

Narrow strips beneath longer hair are shaved and then covered by the hair above.

Fully unshaven / long-hair FUE

In selected patients, grafts can be harvested while the hair shafts remain long. This is technically more demanding and the practical graft number may be more limited.

Who May Be Suitable for an Unshaven Hair Transplant?

  • Men who do not want to cut all of their hair short
  • Female patients
  • Patients who need grafts placed between existing hairs
  • Limited hairline or temple restoration
  • Patients who wish to minimise the visible change after the procedure

Is DHI Better for an Unshaven Hair Transplant?

Yes. In our practice, DHI is clearly the preferred technique for unshaven hair transplantation. It allows more controlled implantation between longer existing hairs and often avoids the need to shave the recipient area. Sapphire FUE can be performed without a full shave in a small number of suitable patients, but it is far less commonly chosen for a truly unshaven procedure.

Advantages of an Unshaven Hair Transplant

  • Preserves hair length
  • Allows a less obvious change when returning to daily life
  • Offers an important cosmetic advantage for many female patients
  • Makes it possible to implant between existing hairs

Disadvantages and Limitations

  • The procedure may take longer
  • The practical graft number may be more limited
  • Technical difficulty increases
  • For large bald areas and very high graft requirements, a full shave may be more appropriate

How Many Grafts Can Be Used in an Unshaven Procedure?

There is no single fixed limit. Unshaven approaches are particularly suitable for limited areas, hairline work, temples and thinning zones where existing hair remains.

Unshaven Hair Transplant for Women

Women are among the groups most likely to prefer this approach. The cause of hair loss and the donor area should still be assessed before surgery is considered.

Unshaven Hair Transplant for Men

It may be considered in men with longer hair, limited hair loss or a strong preference for reducing the visible signs of surgery.

Does an Unshaven Technique Look More Natural?

Not automatically. Naturalness is determined by hairline design, graft angle and direction, the use of single-hair grafts and how density is distributed.

Is an Unshaven Hair Transplant Less Traumatic?

Not necessarily. Leaving the hair long does not mean the scalp itself is not being treated.

How Long Does an Unshaven Hair Transplant Take?

Hair transplant surgery generally takes around 6–8 hours. Working carefully between longer hairs can increase procedure time, and DHI implantation is commonly performed by one operator.

What Will I Look Like Afterwards?

Redness, mild swelling and crusting can still occur. Longer hair may help conceal some of the treated areas, but the procedure is not completely invisible.

Shock Loss

An unshaven technique does not prevent shock shedding. Temporary shedding can occur in transplanted hairs and, in some cases, in some existing hairs.
